On our server B grade is hard to get. There are a few ways:
Craft (probably the easiest, but you need a spoiler and a crafter)
Drop
Buy from other players if you are super lucky
Buy with boxes at caska, its expensive, and I wouldn't recommend buying a whole set this way
Buy with life crystals (which drop from RB's, also hard to do)
Most wait for A grade which they craft/get drops/buy from players/get given to them.
It really does help to have a clan or a group of friends so you dont have to work alone to get things.

event gift boxes. They are tradable for all kinds of useful items at the NPC named Caska in Giran. She's under a tree at the bottom of the stairs near the priest of dawn.

Yep anyone, any class level 60 and above, unfortunately due to some people exploiting, we made it so you have to get at least one kill to get the prize.

ALL the end bosses are worth doing, but not all are super high level.
Queen Ant, Zaken, Antharas, Valakas, Baium are most popular for their jewelry they drop.
Not sure what you mean by "dungeons worth doing".. the big guys, Baium, Valakas, Antharas are achievable through quest or buying the item with boxes.

Yea most have an AIO (all in one) buffer.
Popular classes for this are:
Prophet, Warcryer, Bladedancer, Swordsinger, Elven elder, Shilean elder, Dominator (does nice clan buffs)
You can have one main class and 3 subs, so 4 classes on one character.
So pick carefully, I think it depends on what class you like to play most and what buff befits you want as to how you'd want to set up an aio. You almost need a bladedancer and sword singer no matter what, but careful because Dark Elves cant be Elves and vice versa, so dont start with an elf or dark elf.

40-60 eh...
class would be helpful but here's some tips:
Open your map (alt+m) and you'll see a world info button, under that is lots of usefull information such as hunting zones etc.
With no buffs, I'd say Giran area, Dragon valley entrance, or Dion area Cruma tower first floor
Once you get 50ish, I like Oren area Outlaw forest
Lots of people like Aden area Cemetary too, around level 50 with no buffs, then if you move toward the forbidden gateway as you get 60 those mobs are right for you also.
In all areas just be aware of the agro.
