For problem #2, you're trying to insert the same value, in this case, 0, when it needs to be a unique value. Try just deleting all the 0's that occupy the first column or replace it with NULL. If neither of those solutions work, try deleting the 0's again but this time manually specifying the other 3 columns (aka everything minus the 'id' column) and that'll definitely work.

Sort of like:

INSERT INTO table (column1, column2) VALUES ('a', 'b')
