Aren't we meant to get a warning, varying if we haven't already had a warning beforehand?

I cant figure this complicated infraction system :(!

Say I had no infractions or warnings. I do something bad. I'm meant to receive a warning. I do something else thats bad again. I get an infraction. I do another bad thing so I receive a warning, I fail to comply with the reasons for the warning so I get infracted. It's a tad complicated and I think that;s how it works.
