Emblem problems

I recently started my own server and downloaded a template for the website, i'm having a problem with uploading an emblem, when I click the upload emblem button, the page just refreshes. When i try to start the website through htdocs/clan/index.php I get massive errors

This is the code:

Also when I log in, It doesn't show the "No Emblem" image where it's supposed to show it (In the user panel).
