Report player script

ive saw on some sites a report player script i am wondering how it works is it just a normal contact page of a mpog site or is it fully new private coded and not released ?

More than likely it's just a simple text box with a few query.
Create a new table (or edit an existing), insert reporters character/account name, who he/she is reporting, and the reason why.

That would be actually incredibly simple, just by googling a few things. ;)
