if you have the code all you need to do is copy the code into the query window of SQLyog. be sure to have your antrix database selected. then right-click the query window and select Execute Query->Execute All Queries.
it should be the same for the mangoes database too.

what is the site that creates the code
make sure your getting the right code. if your using the wow vendetta site. after you submit your custom item. there will be 3 options to change the code for the correct database, the default is for fanatik's core, select the antrix option and you should be good to go. "item_template" is a mangoes table from what i've seen. antrix uses "items" as its table.

i have a problem when i create the weapon what id do i put it as just a random number i tried that on my arcemu server and i confirmed it and stuff i got on my server and did .character additem ##### and it didnt work how do i tell if theres an id that works and isnt taken?
