I have been running a server and using QuickWeaponSwitch to change weapons during combat and it suddenly stopped working, I now cannot change weapons during combat. What would cause this problem?

I have been running a server and using QuickWeaponSwitch to change weapons during combat and it suddenly stopped working, I now cannot change weapons during combat. What would cause this problem?
Even if no one knows the solution it would be nice to know if anyone else is having this problem.
