in my Script is localhost...
you have to change this because you have to host your page in a hosting: Hosting: Hosting gratuito, Hosting professionale - PHP MySQL EMAIL IPv6 SSL or AlterVista | Lo spazio web gratis con php e mysql and you have to modify the host. but i cant modify the database name! Help us plz!

the password of sa is your database passwort that you set up by installing MSSQL

localhost = ServerName...your username and possword you can change in security tab in MSSQL/
