[BCStorm} 2 issues, MYSQL error and black client

Hi, I have two problems with my retro.

1: My client loads but it goes black and then i get disconnected.

The SWF's are set up correctly...

I also changed banner.php to banner.png

Also i get a mysql error above all the pages, I have mysql set up properly.
