SEARCHING Table LOGIN ATTEMP

Im putting on the web for mi own server and i take it one web template from a blog of "xfiles20"

that web require a table called LoginAttemp

i anyone had the table please share I need it please ,,,,Thanks for your help

i been created with the solution the table contains three columns
LoginAttempts Table is the name

first column IP varchar(15)
second column Attempts int
third column LastLogin date

Im thinking a more of one person can be util this
