Often crashes upon entering new instances

I am trying to learn how to make my own server and i encounter this problem anyone can explain to me how to fix this ? thank you:blush:

yes, i added teleport from mentus and i started messing up on GUI editor and editing some TGA files from theme, the crash often triggered everytime i enter on instances like dungeon or forsaken tower, could be the problem from splash loading screen? because i didnt crash outside and teleport works fine outside madrigal. anyway thanks for answering. edit : i tried using clean v15 file without any custom system i still getting the same problem :

bump. i tried installing the files from different PC, still, im getting the same exact problem (k15/k18)
