Need working register scrip

I need just a working register script to register accounts in game currently im using

http://24.255.230.37:8080/aionweb/reg.php

but its missing alot of code and includes.

If somone has a working register script can they post here please.

Preferrably one without images so i can just iframe it to that reg.php page

im getting this error at the top of reg page
