Everytime i take a item out of the premium item inventory it stays there.
It still goes to the normal inventory but its also still in the premium item inventory.

Yea, seems to be a common issue. I've tried many things to try fix the issue. It may be the dedi.
Is it all items? Are you able to test for example a Short Sword?
Its with all items. Sometimes I can take 1 Item out properly but the next item i try stays in inventory still.
I currently have the same issue except some items dont even appear in my sc invent until a full restart.
Then it sometimes does the duplication and other times not.
Ive reset dedi, reinstalled server, had countless people try find the issue.
Came to the conclusion that it was the dedi.
You can back them up by right clicking the db>Task>backup.
But I've reinstalled the dedi and it didn't fix it.
Out of curiosity, who is your dedi hoster?
No, I believe the issue is with particular dedi's.
From what I read somewhere on another thread is that it is a common issue on S4Y dedi's for the item malls to not function correctly.
We dont use Server4you so.. o.o

Your issue is linked with caches.
You can see logs of each request parsed and what's being parsed here:
There'll be an Index.dat; within it contains requests that the Login/Character services use; this is your issue as the request about to be sent matches the cache; thus the service doesn't parse a new request and just reads the cache.C:\Windows\System32\config\systemprofile\AppData\Local\Microsoft\Windows\Temporary Internet Files\Content.IE5
We had to fix an issue linked to caches many moons ago; pretty much the services read this before bothering to call for a new request to be sent.
All this is findable with a dump of the services (Login in this case); which has it all stored in memory.
Other resources my Login services was using (for the sake of it; took me a good 10 minutes to dig this up again; forgot where the caches were stored):
