Port forward behind router using ip intercept to host server

nid help i have screen shot
can someone check if my port farward is correct base on my server cgf?
I dont understand because there is service port and control port in my config server..so what should i put in my port forward web?
You can edit my printscreen to correct base on my server config..
Hope somone can help me.. Im using pldt fiber behind router

i also using ip intercept so i can play in same pc but still disconnected..
My ip intercept running okay but i dont know still disconnected...
