You have dll with the source right ?
Search for tutorials how to excute to SQL ODBC.
Connect to MuOnline Database with your dll and excute the skill there .
Every "/addskill" command will excute diff SQL Code.

if you want to make a custom stuff you really need to build a .dll file with a source that to respond to this command /addbuff and to execute it to the server, also you need to make the right check to the server and client to be 1:1
