I need a simple register page for my website

So it's been a longe time since I have stopped to work on Mu Online stuff and now that I am back a lot of things changed, like the PHP+MSSQL connections, so I am looking for a simple registration page to use on the wordpress website I am working on, if you have one to recomend me I'd be very thankful, cya!

Both too old, both have the old PHP+MSSQL functions which don't work anymore, thank you anyway.
