GM Tool - Can't find my server adress and port

I downloaded the GM Tool - TDChien88 at http://forum.ragezone.com/f458/cabal...LCC=3389000673 and when I open it, it asks me for my server adress, port, username and password and I don't know how to find my server adress and port. Can anyone tell me how to find them? Here is a image of the tool.

EDIT: Problem solved. The ip adress was my ipv4 adress and the port was 1433.

O endereço e o IP do SQL e a Porta e 1433 que e a porta padrão se vc não tiver mudado.

Server adress is the IP of the SQL DB - go CMD /ipconfig
port is 1433 by default
