How to creat item with 3 same slot??

I've used balq tool to try to creat item with 3 same slot option
Ex: mithril weapon with 3 slot sword amp

but alway get 3 slot with 1 slot amp and 2 slot clean

anyone here know plz help me

lol, I've told I had used BALQ TOOL

but only 1 , 2 and 4 slot same slot option
can't creat 3 slot same slot option
