How to make WARP?

Hello, I have a question i try to use serach but i cant find good tutorial how to create a new WARP :). I wanna do warp in LS4 what will teleport me to LS5. I try do it alone but all time any error... Its a example, but if i want do other warp meybe in DS in City what will teleport me to hmn.. PF will be do that same ye?

So:
1.Say me what files must be edited
2.How to edit it (How to add a new warp).
