Help me please, i have got a problem eror when create a new character

when a user creates a character, writing error: failure to create character..

3 possible issues

1: Database not set up correctly or corrupted
2: Possible problem with ini file
3: Your database was wiped at one point and you will need to create as many charters as possible until you reach the point where you get a new serial generated.

The 3rd one worked for me but i only needed to create about 6 or so new characters to fix the issue.
