Meh, not necessarily if I remember correctly. It might just require him to actually have a mysql password, although that's really annyoing at times while working locally. Didn't really read too much into the error, but you must be blind getting your Mysql details wrong - especially in this case.

It's okay I guess. Personally I'm not a big fan of staff badges that 1) look unprofessional (in the sense that it's 1 pr staff member) and 2) can be mistaken for another badge. On top of that, some people change their avatar "often", so it wouldn't take long before the badge would be outdated...

Well, if you just want to be secure, download the original Rev 1.9.9.9 or whatever. Patch the forgotten password thingy and don't allow any unescaped input fields (most commonly found in poop themes), and you're good to go. I personally don't use Rev, but if there were more bad bits in it, we...

I like the concept, but personally not a fan of things that might require a rather fast connection to work properly, not to mention phone users. Good work regardless :)
