pwAdmin Connection Refused Error

guys anybody can help me with this error

It tells you right in the error message what went wrong...

I mean how to solve this, I followed tutorial by you correctly in you 1.4.2 guide, and I changed all DB pas to my own, when I open adminer it works perfectly and I can look pw DB.

but when I open pwadmin it only show tab and banner, .
