Clan system 2.0

Hello, I'm having problems with clansystem 2.0 (sandurr), although I have followed all the instructions on the here, but I failed (9) after input name and select create, please help me or someone can upload the 2.0 clan system with no errors!!, thanks!!

I have been using your version uploaded, but I still get error (9) after input clan name and choose to create, here are pictures of my configuration, expect that there will be solutions to the problem I'm having.
