[Help] About Demon Shaitan Dress

I Mad all Step for media and i add only the item in
itemdata40000.txt
everything work good in game, i have only 2 problem :/
with image avatar dress Male , and when i try to Wear, the Attach ( Accessory ) Look This Video and i hope if i get any idea or help to fix it :(

files add in data.pk2 is wrong bro or ID item is wrong too

if u mean the error when add it in _packageitem i fix it too
