i think it's possible. but you need to edit some Files in client. Only in client like Player.bmd or some BMD's
You can make potion girl with red skin from Data/NPC (Potion girl is Girl01.bmd).
About make it only for PK's, you will need to write some codes for your GS (I can't help you here :)).
