Error adding avatar. get in shard cant find check to understand more

Hello.
so i trying to add in avatars and funny part is i get same damn error on all the avatars i adding
I getting it in both SR_GameServer and SR_ShardManager

So now i need ask for help

How will i solve this error ?

i have went in DB on common and item make sure everything is right as the code and they are
But still i getting error.

So any new ides i can try ?

make sure you have add item with last ID from db...
