Special Item 1 (Key issue)

Ok so for some odd reason when i go to use Special Item 1 (Key 4), it does nothing but Special Item 2 (key 5) works perfectly what could be the problem exactly?

I'm assuming you've tried clicking default controls in options > Keyboard?

Check zactioncount and loaddefaultkeysetting to make sure nothing's missing.
