PHPretro Staff Application

[Intufnix]

Hey guys
So i just coded this PHPretro addon, a Staff Application, and i wanna share it whit you.
Tell me what you think about it, let me know if you find any bugs and let me know if you think there is something i should add to it
Ooh, and yeah, i know i typed some of the texts wrong, but that migth be beacuse my "Friend" spilt an half glass of coca cola on my keyboard ._. So its kinda fucked...
Well!
Picture:

You migth wanna know: It is using MySQL and im not done whit the Housekeeping thing for it, so you will have to edit it through PHPMyAdmin. It will send an eMail to you when some one has applyed for a job at your hotel. And you are able to add new ranks if you know how to copy'n'past and do some small edits
And here is a download link: http://forum.ragezone.com/attachment.php?attachmentid=81880&stc=1&d=1287839639
There is an guide in it if you need help whit the setup
